We are seeking a candidate that can provide discharge planning as well as comprehensive medically related social services to our patients.
Responsibilities include MDS, Care Plans, Assessments and timely documentation operating within the framework for the company's standards for best practice in accordance with all state and federal regulations.
The qualified candidates will have a 4-year degree in Social Work.
